| Category | Specify whether you are General customer, NRI or staff of PNB. |
| Card request | Specify whether the request is for New Card or a replacement. |
In case of Replacement please provide following information:

| Previous Card Number | Enter number of your existing card. |
| Reason for replacement | Select reason for replacement from the given options. |
| Reported on | Enter date on which you reported loss of Card to the bank. |
| At | Enter place where you reported loss of Card. |
| Other Reason | If reason of replacement of card is other than options provided please specify here. |
Submit to submit the request form. If the request has been sent, a message indicating the same is displayed on the screen. If you want to check on the sent request, go to the Message Log folder in the MAIL option. Your request will be displayed as a sent mail in the message log list. You can check if the request has been read by your relationship manager.
